Online Forms for B.S. in Commerce Students

Request for Exception to the Academic Rules Committee	Allows McIntire students to request suspensions in the stated rules and regulations. Decisions on the petition are subject to the discretion of the committee.
Major/Minor Declaration Instructions Major/Minor Declaration Interim Degree Audit	Provides the information necessary for a McIntire student to declare a major or minor in a program from another school at the University of Virginia. Required for fourth-year Commerce students seeking to graduate with a major/minor outside the Commerce School.
General Degree Requirement Checklist (2008)	Provides McIntire students with a checklist to ensure that they are meeting the course requirements for the completion of the B.S. in Commerce.
Course Planning Guide	Guidelines for choosing a course of study for McIntire students.
Concentration Change Request	Required to change Commerce concentration.
International Business Concentration Requirements Checklist	Provides students who are concentrating in the area of International Business at the McIntire School of Commerce with a checklist for meeting the requirements of the program.
Leave of Absence Request Form	Allows a student to request a leave of absence from the McIntire School of Commerce.
Request for Readmission	Allows students to request readmission to the McIntire School of Commerce after a leave of absence.
Independent Study Permission Form	Provides McIntire students with the necessary information for completing an independent study for an area. The document also contains the necessary form to apply for permission to complete the independent study.
Transfer Credit Request	Allows students to seek permission to take courses an another university and have them apply toward a degree at the University of Virginia.
